Kralendijk – If you have not yet collected your health card, Care and Youth Caribbean Netherlands (ZJCN) invites you to do so. The health card is intended for residents of Caribbean Netherlands who have been insured with ZJCN for two years. The card replaces the paper insurance certificate and simplifies the healthcare process, especially in urgent medical situations abroad.
To make it easier to collect your health card, you can now visit us during our regular walk-in hours at the ZJCN office at Kaya Grandi 91:
- Monday, Wednesday, and Friday: from 08:00 to 12:00
- Tuesday and Thursday: from 13:00 to 16:00

It is also possible to make an appointment in advance. Send an email to zorgpas@zorgverzekeringskantoor.nl and include your full name, ID number, and phone number. We will contact you as soon as possible to schedule an appointment.
What should you bring?
To collect your health card, you will need a number of documents. Bring your ID and a completed customer form. This form can be found at www.zvkcn.nl or at our office. Parents or guardians can collect the health card for minor children; a passport is also acceptable for minors.
If you are unable to come by yourself, someone else can collect the health card on your behalf. The authorized person will need the following documents:
- A signed authorization form, signed by both you and the authorized person. This form can be found at www.zvkcn.nl or at our office.
- Your original ID.
- The identification of the authorized person.
Important to know
The health card is not a replacement for travel insurance. Therefore, do not forget to take out additional travel insurance when traveling abroad. This ensures that you are fully covered in medical emergencies.
