Residents can be referred to a well-being coach, for example, by their GP, practice nurse, or district nurse. They can also contact staff at the Social Team directly. During an introductory meeting, wishes, interests, and possibilities are discussed. The well-being coach thinks along and helps find a suitable activity. If desired, the coach can support the first steps, for example by accompanying someone to an activity.
The support is provided by staff from the Social Team and Goed Bezig. This support is free of charge. Within a week of the referral, the well-being coach will contact the resident to make an appointment.
